- You must be 18 years of age to participate and nominate anUltamate Mom
- All submission including the Mom's name you are nominating will be posted here in ultamatemom.com
- The person you are nominating has to be a Chicagoland resident and has means to to travel to and from the Salon at ULTA.
- The winner must be able to attend the "Day of Beauty" @ Salon at ULTA on May 9, 2009 - 9:30am. 114 S State St. in Downtown Chicago. Must be on time!
- There is no cash value to the prizes.
- All photo sessions will follow right after the "Day of Beauty" and will take place in the Salon at ULTA.
- Non-transferable. You must attend the scheduled appointments to receive the prizes. Your prize will be forfeited if you do not attend.
- All entries will be announced in our Twitter and Facebook accounts including the Mom's name you nominated.
- You must attach the most recent close-up picture of the person you are nominating when submitting written entries with your full entry.
- Your video must have an answer to all the questions listed in the How To page at the very beginning of your video.
- Your video must be loaded up in You Tube and the url link to the video is all that we need. Do not send a raw video file directly.
- You must respond within 24 hours or we will call the next person in line and your prizes will be forfeited.
- The 2nd and 3rd prize winners must choose which 2 services they want to receive. Choose Choices are: hair cut, hair color, or facial.
- Please bring a change of clothes for the photo session
- The Photo Session with lahleyoo! will begin as soon as your services at the Salon is finished.
- The Photo Session will take place in the salon.
- Will be mailed to you directly in 7-14 days after
